Ever had one of those days when a patient’s request gets completely garbled? Just yesterday, a patient asked for their ‘ear wax report’ instead of their ‘lab report’. I mean, who knew that was a thing? It’s little misunderstandings like these that keep our days interesting, but they also remind me how crucial it’s to maintain data accuracy while navigating the world of medical records.

I totally get that! Once, a patient asked for their ‘nurse’s notes’ instead of ‘medical history,’ and it took a while to figure it out. Having a solid protocol for confirming requests, like repeating them back, can really help avoid these tongue twisters.
