Streamlining chart prep across shifts

Quick question: what tools or templates are you using to keep patient charts consistent across shift changes? On our medical unit I built a chart prep smartphrase and use a shared inbox with color flags for labs/consents; it cut our 2 pm backlog from 18 to 6, but radiology orders still get buried without a clear owner… Curious how you route those to the right team without pinging five people.

And which EHR are you on? We solved the “no owner” problem by auto-routing all imaging orders into a Radiology Orders pool with a rotating owner-of-day, and our chart-prep smartphrase drops “ROD: Name — date” at the top so anyone can see who’s holding the baton. If your system allows rules, split STAT vs routine to separate subpools so STAT never waits behind routine.

We fixed the “no owner” issue by adding a required order question — Responsible service — and mapping each answer to a team InBasket, then a 1:45 reassign rule moves anything still unread to a backup pool so your 2 pm backlog doesn’t swell. I also tucked a tiny stamp in my chart-prep smartphrase: IMG [modality]/[needed-by]/[owner], so the next shift sees who’s on it without a ping. Same issue with ‘radiology orders still get buried’ — we fixed it with an Imaging Triage workqueue that catches orders with no service and shows an aging timer; at 20 minutes it auto-reassigns to the CT/MRI/US lead on duty with a red age flag, no pop-ups. With your 2 pm backlog, we also run a 1:40 sweep that surfaces anything >60 minutes to day-shift triage so it doesn’t sit in a general pool. Map CPT ranges to modality queues and turn on the aging column to keep ownership clear without pings.

Quick example: I cut the ‘radiology orders still get buried’ issue by adding the modality as a temporary care team member at order entry (CT/US/MRI), which auto-routes those orders to the correct team InBasket without paging. Since you’re already using a smartphrase, add a one-click step that both adds the care team member and drops “Owner: CT” in the Order Comments for cross-shift visibility; the only caveat is giving float staff a two-minute refresher.
